This month I would like to encourage you to read a very interesting article titled, Palliative Care and Advanced Cardiovascular Disease in Adults: Not Just End of Life Care: A Scientific Statement from the American Heart Association, published on the 17 April 2025. https://www.ahajournals.org/doi/10.1161/CIR.0000000000001323

The article highlights that even though patients with advanced cardiovascular disease may be living longer there is often an adverse impact on their quality of life, reinforcing the need for palliative care services and support earlier in their trajectory then what commonly occurs now.

This statement also reminds us that palliative care is not just about end of life care and there is a great opportunity to focus on patient’s quality of life too when palliative care is introduced early.

This scientific statement:
- ・Highlights the importance of effective communication including shared decision making, and advance care planning
- ・Provides an overview of recent evidence related to the palliative care including symptom management, and quality of life
- ・Discusses diversity, equity and inclusion aspects
- ・Discusses ethical and legal concerns related to implanted cardiac devices
- ・Provides strategies for palliative care engagement
The article suggests in advanced cardio vascular disease there are 4 essential items that are inter—related including the patient and family, our communication, shared decision making, age appropriate care and advance care planning. The authors have developed the below figure to provide an explanation as to how these components support the essential palliative care domains of symptom management, quality of life, spiritual and psychological care and bereavement support when completed as part of our clinical practice.

The article goes on to provide suggestions on the strategies we can utilise for communicating with patients and families across the advanced cardiovascular trajectory. Refer to the below figure as a reference for these suggestions.

The article highlights many interesting aspects of advanced cardiovascular disease in adults and I would encourage you to read the article and reflect on any opportunities that may present to improve your practice for this client group.
