このウェビナーはアメリカから発信されていますが、緩和ケアと心不全に関する情報は、私たち全てに関連するものであることにご注意ください。

 

このウェビナーでは、次のような内容が取り上げられました:

  - 心不全患者の疾患パターンについての注意喚起

  - 臨床医が予後や治療効果を過大評価することがあること

  - 症状や将来の治療法について、患者さんやご家族と積極的に話し合うことの重要性

  - アドバンス・ケア・プランの重要性

  - 緩和ケアへの紹介のきっかけ:症状が管理されていないとき、ケアの目標について     

   話し合う必要があるとき、治療がうまくいっていないとき、負担が大きくなって    

   いるとき、ステージDのときなど。

   – ゴール・オブ・ケアや予後に関する会話は、臨床医に特別なスキルセットと快適な 

   レベルが要求されるため、緩和ケアのコンサルテーションは非常に有益である。

 

ウェビナーは英語で行われますが、繰り返し見ることができます。 また、このセミナーを受講して、どのようなことが

分かったか教えてください。

  • This month I would encourage you to review a free webinar which has been recorded by the National Hospice and Palliative Care Association in conjunction with the American Heart Association titled “Heart Failure and Palliative Care:  When to Engage”.

Reminder, the webinar is coming from a United States of America perspective, however the information related to palliative care and heart failure

is very relevant to us all. 

 

The webinar covered the following areas:

 

  •   *Reminded us of the pattern of disease for heart failure patients
  •   *How clinicians at times over estimate prognosis and benefits of treatment
  •   *The importance of proactive discussions with patients and families in relation to symptoms and possible treatments
  •         in the future
  •   *Importance of advance care planning
  •   *Triggers for referral to palliative care including; when symptoms are not being managed, goal of care discussions are
  •         needed, treatment is not working or becoming more burdensome, and when in stage D.
  •   *Conversations on goals of care and prognosis require a specific skill set and comfort level by the clinician, which is
  •         where a palliative care consult is very beneficial
