  • Following on from last month’s focus on the importance of communication and consistency in language, let us expand this further this month to reflect on the importance of communication for patients with Advanced Heart Failure and their families.  Let’s look on how we can improve our practice to ensure the best possible end of life care.  It is essential that everyone on the care team, including patients and their families, have a shared agreed understanding of the goals of care through effective communication with the health care team. 

    The following articles are provided to enable us to start to ponder what we can do better:

  •    –  Inadequate Communication Exacerbates the Support Needs of Current and Bereaved Caregivers in Advance Heart Failure and Impedes Shared Decision-making,
  •       D Fitzsimons, L Doherty, M Murphy, L Dixon, P Donnelly, K McDonald, S Mcllfarick, 2019).
  •       The 3 main themes discussed include poor communication, living with uncertainty and lack of service provision.  It is interesting to read the subthemes which provides us with an in-depth understanding of the               impact on the carer.  The use of direct quotes is an impactful way of ensuring we understand the impact from the individuals perspective.

  •       This article involves a qualitative study with 23 Physicians who care for patients with AHF exploring their perception of the characteristics of high quality EOL care and the barriers encounted.  
  •       Whilst a small study it provides some insight into the varying views of the physician’s perspectives in this area.

  •    –  Factors Influencing Potentially Futile Treatments at the End of Life in a Multiethnic Asian Cardiology Setting: A Qualitative Study, 2021, J :p. S Yoon, S Neo, D Kheng, N Graves.  
  •       The article identified three major themes regarding factors that the participants reported to influence the decision making and provision of medically futile treatment among cardiology patients:
